In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: Record review revealed that the facility had a surety bond in the amount of $7500.00. Review of the bank statements for the resident trust account for the last three months (July-September (YEAR)) revealed that on 33 of 92 days, the balance of the resident funds account exceeded the $7500.00 covered by the surety bond. During an interview on 10/3/2017 at approximately 3:00 PM, the manager responsible for handling resident personal needs funds was unaware that the surety bond did not meet the requirement. You are hereby formally notified that where the above listed deficiencies also constitute non-compliance with applicable provisions of the 'Rules and Regulations for Licensing of Nursing Facilities' they are deficiencies under State Regulations and grounds for licensure sanctions. 2020-09-01
