In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: Surveyor observation on 3/4/2019 at 1:38 PM revealed that Resident ID #59 has his/her own natural teeth. Review of the clinical record revealed this resident was admitted to the facility on [DATE]. Further review revealed a 9/8/2018 annual Minimum Data Set assessment stating that the resident has obvious or likely cavity or broken natural teeth. There lacked evidence that the resident had obtained routine dental services. During a surveyor interview with the Charge Nurse on 3/7/2019 at 9:14 AM, she was unable to provide evidence that the resident or responsible party declined routine dental services. During a subsequent interview with Resident ID #59's responsible party on 3/7/2019 at 10:56 AM she revealed that she would like the resident to be seen by a dentist. 2020-09-01
