In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The pharmacy recommendation states, If the medication cannot be discontinued at this time, current regulations require that the prescriber document the intended duration of therapy, and the rationale for the extended time period. The report is signed and dated by a prescribing practitioner on 12/11/2017. Additionally, handwriting on the bottom states, Pt (patient) is on Hospice-will not change any meds. During a surveyor interview with The Cove Unit Manager on 8/09/2018 at 2:52 PM, she was unable to provide evidence of the intended duration for the [MEDICATION NAME] order. 2020-09-01
