The facility's policy titled, Medication Administration Methods states in part, 4. Medication administration must be documented on the Medication Administration Record/ EMAR immediately before going on to the next resident, documentation will include: .d) The nurse will take the pulse or blood pressure when a medication requires such. He/she will then record the results on the Medication Administration Record/ EMAR prior to medication administration . Findings are as follows: Clinical record review revealed a physician's orders [REDACTED]. Hold for systolic blood pressure (SBP) less than 110. Clinical record review revealed a physician's orders [REDACTED]. Give 1 tablet by mouth in the morning for Hypertension Hold for SBP less than 110 and heart rate (HR) of less than 60. Record review of the EMAR from 7/19/2019-10/31/2019 failed to reveal evidence of blood pressures being taken prior to medication administration on the following dates: 7/26, 7/27, 7/28, 7/29, 7/30, 7/31, 8/10, 8/11, 8/16, 8/17, 8/18, 8/23, 8/24, 8/25, 8/26, 9/18, 9/19, 9/20, 9/21, 9/22, 9/23, 9/25, 9/26, 9/27, 9/28, 9/29, 9/30, 10/16, 10/18, 10/19, 10/20, 10/21, 10/24, 10/25, 10/26, 10/27, 10/28 and 10/29/2019. During surveyor interview on 11/01/2019 at 2:14 PM with the Director of Nursing, she was unable to provide evidence that the blood pressures were taken prior to administering the above medications. | 2020-09-01 |
