In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are as follows: Clinical record review for Resident ID #45 revealed a current physician's orders [REDACTED]. Further record review revealed a pharmacy recommendation report stating, The resident has an order for [REDACTED].PRN psychoactive medications may only be authorized for up to 14 days, then the resident must be re-evaluated for appropriateness and if continuation is necessary there must be a determined length of treatment. This recommendation was signed off by the Physician on 10/11/2018 indicating the Physician disagreed with the recommendation; however, he documented no rationale for extending the order or a length of treatment.
