In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based on observations made during the FOSS recertification survey and group interview, it was determined that the facility failed to ensure that linens are in good condition. This could affect 4 of 14 admitted residents. (Rooms #101-A, #102-A, #104-A and #105-B) Findings include: 1. During the initial tour performed on 5/11/17 at 9:20 am and physical environment with Physical Plant Supervisor (employee #4) performed on 5/11/17 at 10:23 am it was found that rooms #101-A, #102-A, #104-A and #105-B the linen was observed with holes. Interview performed on 5/11/17 at 1:45 pm to Director of Nursing (employee #2) reveals that the facility have a contract for the laundry. Our personnel have to check the sheets before dress the beds. Awe do not have at this moment inventory of new sheets. |
