In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based on review of the facility's in-service training for Emergency Preparedness during the rectification FOSS extended survey, it was determined that the facility failed to ensure that staff and employee are trained in emergency procedures and equipment to work in the facility which could affect 13 out of 13 admitted residents. (R#1 to R#13) Findings include: During the review of the facility's training manual, documentation and observation made of the physical environment on 5/24/17 at 2:45 pm, no evidence was found that nursing staff and employee are trained and competent related to defibrillator use in the event of an emergency and Green Code. |
