In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based on a recertification FOSS survey, tests, observations, review of documentation policies/procedures and interviews with personnel and residents made during the physical environment survey, it was determined that the facility failed to ensure a safe and functional environment affecting 13 out of 13 admitted residents (R). (R#1 to R#13) Findings include: 1. During observations of the physical therapy room on 5/24/17 from 9:45 am through 11:15 am it was found: a. Bathroom of Room # 307 is was observed in the bath tub area a hole in the shower. Painting the bare walls. Accessories deteriorated. b. Resident Room bathroom #308 accessories deteriorated and putty on the wall exposed. c. Resident room #308-B the closet door is uneven and not permit a complete close. d. Residents' room #318 the nursing call system lamp in front of the door do not have the cover. e. Bath floor tiles with old stains in resident rooms # 309, # 310, # 311, # 315, # 316 and # 317. |
