This deficient practice was identified in 2 out of 16 active cases reviewed. (RS #231 and #233). Findings include: 1. Resident # 231 is a [AGE] years old male admitted on [DATE] with a [DIAGNOSES REDACTED]. During the record review performed on 05/21/19 09:48 AM provide evidence that the Resident have high risk medication orders [REDACTED]. No evidence was found that the License Pharmacist performed a Medication Regimen requirement in order to identify the [MEDICAL CONDITION] drugs are not given these drugs unless the medication is necessary to treat a specific condition as diagnosed and documented in the clinical record. 2. Resident # 233 is a [AGE] years old female admitted on [DATE] with a [DIAGNOSES REDACTED]. During the record review performed on 05/21/19 10:29 AM provide evidence that the Resident have high risk medication orders [REDACTED].
