In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This deficient practice had the potential to affect 3 out of 3 (Resident #1, #2, # and #3).admitted residents from which the right to examine the results of the most recent survey and the plan of correction in effect was not promoted. Findings include: During the initial observational tour of the facility on 3/14/17 at 6:55 am, no evidence for the initial state survey performed on (YEAR) and the plan of correction for the deficiencies. The facility must make the results available with all the deficiencies and with its approved plan of correction for residents' examination. 2020-09-01
