In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

> Based on interview and record review it was determined the facility failed to use the services of an RN during a 24 hour period for 24 of 86 days reviewed for RN staffing. This placed residents at risk for unassessed needs. Findings include: A review of the Direct Care Staff Daily Reports for the months of 12/2018, 1/2019, 4/2019 and 5/2019 revealed the following days with no RN coverage during the 24 hour period: December (YEAR): 12/11, 12/12, 12/19, 12/22 and 12/24. January 2019: 1/5, 1/7, 1/8, 1/9 and 1/15. April 2019: 4/7, 4/8, 4/9, 4/11, 4/16, 4/23, 4/28, 4/29 and 4/30. May 2019: 5/5, 5/7, 5/11, 5/14 and 5/19. On 6/5/19 at 11:23 AM Staff 1 (Administrator) acknowledged the lack of RN coverage for 24 out of the 86 days reviewed and indicated this was an area they were focused on but were having a difficult time with recruitment. |
