In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This placed residents at risk for unmet medical needs. Findings include: Resident 15 admitted to the facility in 5/2010 with [DIAGNOSES REDACTED]. A 6/20/17 physician's orders [REDACTED]. The 7/2017 MAR indicated [REDACTED]. On 7/31/17 at 10:17 am Staff 10 (RN) stated the medication was available in the medication cart. On 7/31/17 at 11:32 am Staff 8 (RNCM) stated Resident 15's medication was available in the medication cart and there was a backup supply of the medication available as well.
