In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This placed resident's at risk for food-borne illness. Findings include: 1. On 5/31/16 at 7:57 am nine containers holding freezer burned food (freezer burn is a condition that occurs when frozen food has been damaged by dehydration and oxidation, due to air reaching the food) were observed in the upright freezer. On 5/31/16 at 7:57 am Staff 11 (Dietary Manager) confirmed food in the nine containers had frost on them. She stated she thought the food appeared to have been put in the freezer while still warm. 2. On 6/2/16 at 6:55 am the upright freezer was observed to have seven containers of freezer burned food as well as two packages of expired egg substitution product. On 6/2/16 at 6:56 am Staff 12 (Kitchen Aide) confirmed all containers in the upright freezer contained freezer burned food. She also confirmed the two packages of egg substitution products expired on 1/31/16. 3. On 6/2/16 at 7:17 am the ice machine was observed to have a brown residue on the inside rim directly above the ice. Staff 11 (Dietary Manager) acknowledged the brown residue inside the ice machine. On 6/2/16 at 8:14 Staff 7 (Maintenance Supervisor) acknowledged the brown residue inside the ice machine and confirmed it could contaminate the ice in the machine. 2020-09-01
