In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on observation, interview, record review, and document review the facility failed to ensure a physician's order for a medication was followed for one unsampled resident (Resident #17). Findings include: Resident #17 Resident #17 was admitted on [DATE], with [DIAGNOSES REDACTED]. The admission orders [REDACTED]. On 5/3/17 at 9:16 AM, during a Medication Administration Pass observation, a Licensed Practical Nurse (LPN) prepared and administered the resident's medications including Vitamin D 400 units two tablets by mouth. On 5/3/17 at 10:55 AM, the LPN confirmed the observation and acknowledged the resident should have received Vitamin D3 2,000 units per the physician's order. The LPN indicated the physician's order was not followed. On 5/3/17 at 12:25 PM, the Director of Nursing (DON) explained the nurses were expected to administer the medication per the physician's order. The facility's policy titled Administering Medications revised on (MONTH) 2012, indicated medications must be administered in accordance with the orders. |
