In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based on interview and record review the facility failed to ensure a Certified Nursing Assistant's annual performance evaluation was completed for 1 of 11 sampled employees (Employee #6). Findings include: On 02/11/2020, during the Personnel Record Review, the personnel record for Employee #6 with a start date of 02/08/18, lacked documented evidence an annual performance evaluation was completed for 2019. On 02/11/2020 at 11:35 AM, the Administrator confirmed an annual performance evaluation had not been completed for Employee #6 for calendar year 2019. |
