In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based on record review and interview, the facility failed to ensure that all employees working with Dementia (a disorder of the brain affecting a persons ability to think and remember and can also affect a person's daily functioning) residents received training at least once per year. This deficient practice has the potential to effect all 126 residents identified on the alphabetical resident census provided by the Administrator on 03/05/18 by not preparing employees for the challenges of working with demented residents, causing higher staff burnout and residents not having their needs met. The findings are: [NAME] During record review of the employee training's it revealed that five employees (CNA #s 8, 15, 16, 17 and 18) did not receive dementia training. B. On 03/12/18 at 3:45 pm, during an interview with the Administrator, she stated that only 75% of the staff that work with residents at the facility have had dementia training. |
