In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This deficient practice likely prevented the 99 residents identified on the facility census list provided by the Administrator on 10/21/18 to have access to accurate staffing information. The findings are: [NAME] On 10/27/10 at 2:30 pm during an observation the Daily Staff Posting, on the Wing 3 Hallway on the left wall approximately 5 feet above the floor, was noted to be for the dates of 10/26/18 to 10/30/18. The numbers of RN's, LPN's and CNA's had been pre-populated for this 5 day time period with no modification made as to actual numbers of staff on shift. B. On 10/27/18 at 3:10 pm during an interview the Assistant Director of Nursing (ADON) stated that the Daily Staff Postings are completed ahead of the shifts for all shifts. The Director of Nursing (DON) confirmed that the Daily Staff Posting were not done within 2 hours of the beginning of a shift but sometimes as much as 2-3 days ahead. The DON and ADON also confirmed that the location of the Daily Staff Posting was not readily accessible to all residents, family members or visitors. 2020-09-01
