In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based on record review and interview, the facility failed to use employee evaluations for part of the 12 hours of annual education for CNA staff. This has the potential to affect all CNAs that work at the facility (CNAs were identified by the staff schedule provide by the DON on 07/15/17). If the facility is not using the employee evaluations for part of the 12 hours of annual training staff may not be learning for their mistakes and continue to do them, therefore negatively impacting on the care provided to the residents. The findings are: [NAME] Record review of CNA employee training sheets reveled the facility uses an online training format. B. On 07/18/19 at 4:21 PM, during an interview, the DON was asked if the facility uses the employee evaluations for apart of the 12 hours annual training. He stated, That is something I don't think I can provide to you, a link from evaluations to the 12 hours of training. The DON stated that because if there was something that need to be corrected the facility does not wait for the employee evaluations. |
