In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

> Based on record review and interview, the facility failed to submit a Follow-Up Report within 5 working days of the date the Incident Report was filed for 1 (R #1) of 3 (R #'s 1, 4 and 5) residents who had Incident Reports filed with the Department of Health (DOH)/Incident Management Bureau (IMB). This deficient practice has the potential for resident issues that have been reported, to not be addressed and/or rectified in a timely manner. The findings are: Findings for R #1: [NAME] Record review revealed that the facility submitted an Incident Report (self-report) to the Department of Health (DOH)/Incident Management Bureau (IMB) on 8/10/19 regarding neglect. B. Record review revealed that the 5 day follow up for the incident (on 08/09/19) was submitted on 09/09/19. C. On 01/13/20 at 3:50 pm, during an interview, the Administrator reported that she emailed the 5 Day Follow-Up Report to the DOH/IMB on 09/09/19 indicating that she found during an audit, that there was no evidence that the 5 Day Follow-Up Report was sent to the DOH/IMB. D. On 01/13/20 at 4:25 pm, during an interview, the Director of Nursing (DON) reported that she usually submits all of the Incident Reports and 5 Day Follow-Up Reports to DOH/IMB, but they were in-between DON's in (MONTH) 2019 when the Incident Report for R #1 was submitted and the 5 Day Follow-Up Report should have been submitted timely, but it was not. The DON reported that the Assistant Director of Nursing (ADON) and the Administrator serve as a back-up to her when she is unable to submit the reports. The DON reported that she officially started in the DON capacity at this facility in (MONTH) 2019. |
