In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based on interview and review of the facility policy, it was determined that the facility failed to include two of the seven required components that assure the facility is doing all that is within its control to prevent occurrences of abuse and neglect. Findings include: Review on 9/21/17 of the facility policy for abuse and neglect titled Resident Abuse, Neglect, & Exploitation dated 5/31/2017 revealed that of the seven required components that must be included in the facilities abuse policy two were not included. The components are: Screening, training, protection, prevention, identification, investigation, and reporting/response. The two components that were not included in the facility's policy are screening and identification. Interview on 9/21/17 at approximately 2:10 p.m. with Staff B (Director of Nursing) confirmed that the two components had been left out of the current policy when it was put into effect on 5/31/17. |
