In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on record review and interview the facility failed to develop and implement a baseline care plan for each resident that includes the instructions needed to provide effective and person-centered care of the resident that meet professional standards of quality care within 48 hours of admission for one resident in standard survey sample of 43 (Resident identifier is #194). Findings include: During recertification survey on 3/4/18 and 3/5/18 Resident #194's medical record was reviewed. In this review it was identified that the resident was admitted to the facility on [DATE]. Review of the care plan section of the medical record given to this surveyor for review by Staff M (Assistant Administrator) identified that the first care plan that had been developed for Resident #194 had been developed and initiated on 2/13/18 In addition to being late this initial baseline care plan did not include any reference to dietary orders or social services involvment. Interview with Staff M on 3/5/18 confirmed that the document given to this surveyor was a copy of the initial care plan. |
