In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The facility sample was 33 with a census of 41. This was in violation to Licensure Reference Number 12-006.06C. Findings are: Interview with Resident 54 on 04/01/2013 at 1:00 PM revealed the facility failed to inform the resident of the right to file a complaint with the state survey agency and the manner in which that could be done. The resident stated that no information had been given to the resident to report a complaint to the state agency. The resident was not aware of a phone number posted in the facility where the state agency could be reached. .	2016-07-01
