In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Facility census was 37. Findings are: During initial tour observations conducted on 3/19/13 at 11:30 a.m. revealed the state survey results were attached to the wall by the Social Services office. The observation revealed there was no signage or posted information directing residents and/or family members to the location or identifying the documents as state survey results. Observation on 3/21/13 at 9:30 a.m. revealed the state survey results remained attached to the wall by the Social Services office without any signage or posted information directing residents and/or family members to the location or identifying the documents as state survey results. Interview with the Social Services Director on 3/21/13 at 9:30 a.m. confirmed the survey results were not identified by posting or signage to direct residents and/or family members to the location of the survey. 2016-07-01
