In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The failure could potentially prevent all residents, families, and visitors from reviewing facility direct care staffing hours worked. Facility census was 85. Findings are: Observation on Monday 5/20/19 at 8:00 a.m. revealed the Direct Care Staffing Hours form was posted on the 100 wing bulletin board. Further examination of the document revealed the staffing hours posted were for Friday 5/17/19. Interview with the facility Administrator and Director of Nursing on 5/20/19 at 8:20 a.m. verified the bulliten board at the front entry was the area the facility posts its daily staffing hours. The Administrator and Director of Nursing verified the form posted on the bulliten board was dated 5/17/19 and the hours worked by direct care staff had not been posted since Friday 5/17/19. 2020-09-01
