In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The facility staff reported a census of 129. The findings are: Review of current physician orders [REDACTED]. Review of the meal intake documentation revealed that Resident 4 consumed 25-75% of meals. Review of Minimal Data Set (MDS: a federally mandated comprehensive assessment tool used for care planning) dated 2/18/2019 revealed a functional status of supervision for eating. Record review of the current CCP (Comprehensive Care Plan) dated 1/6/19 and updated 2/20/19 revealed that Resident 4 was tube feeding dependent and NPO (Nothing by Mouth). Interventions included: 1. Calorie/Protein/Fluid needs will be met with tube feeding regime. 2. NPO per doctors' orders. 3. Provide tube feeding per doctors' orders. The CCP did not include the current dietary status for Resident 4 of Pureed Diet with Honey Thickened Liquids. Review of the Policy and Procedure dated (MONTH) (YEAR) revealed Assessments of residents are ongoing and care plans are revised as information about the residents' condition changes. Interview on 3/13/19 at 1:11 PM conducted with the Director of Nursing confirmed that the CCP should have been revised to include current dietary status for Resident 4. 2020-09-01
