In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This had the potential to affect all facility residents. The facility census was 68. Findings are: Observation during all days of the survey revealed plastic frames containing two forms. The form facing the nurses station was titled Staffing Daily Schedule and listed the names of staff members working that day organized by units and shifts. The form in the reverse side of the plastic frame was titled by the name of the unit it was posted on and was organized by shifts and job titles with a blank space for hours. None of the blank spaces for the hours had be filled in. Interview with the Administrator on 7/28/14 at 11:22 AM revealed that staff were supposed to put the hours worked on the form every shift and that it should be facing the public and not that staff when completed. 2017-11-01
