In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Facility Census was 123. Finding are: An observation in the bathroom of Resident #77 and #324 on 9/24/19 at 2:28 PM revealed that the toilet riser was stained and the toilet had numerous areas of dried on feces. An observation and interview with the facility Administrator on 9/30/19 at 2:10 PM confirmed there was dried feces on the toilet riser, and the toilet itself has BM in it. This was in the bathroom that is shared by Resident #77 and Resident #324.	2020-09-01
