In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

B. Interview with Resident 4 on 6/20/2016 at 3:15 PM revealed being unable to read the nurse staffing where it was posted. C. Interview with the DON (Director of Nurses) on 6/21/2016 at 8:40 AM revealed the form had always been there. D. Interview with the Administrator on 06/21/2016 9:10 AM revealed the nursing posting of hours needed to be moved. 2020-02-01
