In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Sample size was 1 and the facility census was 76. Findings are: Interview on 3/27/17 at 9:45 AM with Resident 5's family member, responsible for financial's, revealed that information of items covered and items not covered was not provided upon Resident 5's eligibility for Medicaid. Resident 5's family member revealed that it was a long process and that the Medicaid had just started, and that they were not provided with information regarding Medicaid covered and non covered items. Interview with the facility Social Services Director (SSD), on 03/29/2017 1:52:25 PM, revealed that Resident 5's family was assisted with the Medicaid application process. The SSD confirmed that Resident 5's family did not receive a review of Medicaid covered and non covered items, and it was not reviewed with resident or family, when the resident became eligible for Medicaid.
