In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings On 5/29/19 at 9:30 AM an observation revealed an unknown resident exit the West St Jane's entrance un accompanied and wheel self down the sidewalk next to the street smoking. On 05/30/19 06:50 AM an observation revealed Resident #155 exit the front door un accompanied and wheel self to the southwest parking lot and proceed to smoke On 06/04/19 10:05 AM an interview with the ADM (Administrator) confirmed the policy; Tobacco-Free Campus Areas, dated 9/17/2010 was a[NAME]campus smoking policy and did not specifically address St[NAME]residents. Record review of facility policy Tobacco-Free Campus Areas dated 9/17/10 revealed no documentation on how the facility will assess or protect residents that smoke from accidents related to smoking. On 05/30/19 at 3:00 PM the DON (Director of Nursing) provided a list of smoking residents that included; Residents #8, 23, 27, 62, 95 and 155. 2020-09-01
