In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This practice has the potential to affect 27 of 69 residents who used a wheelchair for mobility at the facility. The facility had a total census of 69 residents. Sample size was 15. Findings are: During the resident tour on 7/19/2010 and 7/21/2010 at 9:05 AM found the following wheelchair arms that were non cleanable and breaks in the arm coverings: -Resident 5's wheelchair arm, -Resident 27's wheelchair arm, -Resident 58' wheelchair arm, -Resident 10's wheelchair arm, -Resident 13's wheelchair arm, -Resident 2's wheelchair arm, -Resident 4's wheelchair arm, -Resident 45's wheelchair arm, -Resident 47's wheelchair arm, -Resident 35's wheelchair arm. Interview with the DON (Director of Nurses) on 7/22/2010 at 5:00 PM stated "they had worked with the therapy department on this issue and did not know where they were on parts but may have to order more parts". Interview with the Therapy Person on 7/22/2010 at 4:10 PM revealed the department had been working with nursing and probably needed to order more parts. 2014-03-01
