In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

LICENSURE REFERNCE NUMBER: 175 NAC 12.006.05(11) Based on observation and interview the facility failed to ensure that the state agency survey results book was available for public viewing. This had the potential to affect all residents that resided in the facility. The facility census at time of survey was 159. Findings are: Observation was conducted by the state surveyors on March 1, 2011 for the book containing the results of the most recent survey of the facility. There was no book, or signage indicating where the survey results book was located, in either the front entry, dining room or sitting area of the facility. Inquiry was made to Acting DON of the book location and the Acting DON was unable to locate the survey results book. Interview on 3-1-11 at 12:40 PM with the Acting DON and Interim Administrator confirmed that the survey result book was not located anywhere within the facility; thus the survey results book was not available for public viewing. |
