In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This potentially affected all 52 residents who currently resided in the facility. Findings are: A. Observation on 4/5/11at 8:45 AM revealed the Nursing Staffing sheet was not posted. B. On 4/6/11 at 8:00 AM the Nurse Staffing sheet was posted on a clipboard next to the main lobby area of the facility. The names of the staff on duty in the traditional area of the facility were identified, however the actual hours worked and the total hours were not posted. In addition, staff members on duty in the Alzheimer's Care Unit (ACU) were not identified on the Nurse Staffing sheet. C. On 4/7/11, 4/11/11, and 4/12/11 at 8:00 AM the Nursing Staffing sheets were not posted. D. Interview with the Director of Nursing on 4/12/11 from 2:45 PM until 2:50 PM verified the hours had not been posted as required.
