In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This had the potential to effect all residents that resided in the facility. The facility census was 143. Findings are: Record review of the book containing the results of the most recent survey of the facility conducted by the State surveyors revealed the presence of the statement of deficiencies that resulted from the last annual survey and complaint investigation of the facility on 5/14/09. The book did not contain statements of deficiencies which resulted from past substantiated complaint investigations which were conducted in the facility on 4/19/10, 5/17/10 and 6/23/10. Interview on 8/10/10 at 9:15 AM with the facility Administrator confirmed that the survey result book did not contain the statements of deficiencies which resulted from past substantiated complaint investigations which were conducted in the facility on 4/19/10, 5/17/10 and 6/23/10. 2014-09-01
