In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings are: Observation on 3/15/2011 at 8:30 AM revealed a sign posted on the counter at the receptionist station, identifying that the state survey results were available for viewing upon request. Observation on 3/16/2011 at 8:10 AM revealed the posted sign on the receptionist counter remained indicating the the survey results were available for viewing. The survey results were not available to residents without the need to request. An interview was conducted on 3/16/2011 at 8:10 AM with the facility Administrator. During the interview, the facility Administrator confirmed the survey results were not readily accessible to the residents. 2015-01-01
