In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This had the potential to affect all residents and visitors that could not reach 5 fee and 10 inches in height. Facility census was 26. Sample size was 10. Findings are: A. Entrance into the faciliy on 7/13/2011 at 8:00 AM found the most recent survey results of the faciliy in a file box 5 feet and 10 inches off the floor by the activity office. B. During the General Observation tour of the facility on 7/14/2011 between 9:00 AM and 9:30 AM the Last Survey Results were no visible and accessible to the residents or families. The notebook with the the last survey results were in a file box by the activity office 5 feet and 10 inches from the floor. C. Interview with the Maintenance Director on 7/14/2011 at 9:30 AM confirmed the most recent survey results were located in a file box by the activity office 5 feet and 10 inches off the floor not accessible to the residents and/or families. 2015-02-01
