In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Facility census was 45 and this failure had the potential to affect all residents of the facility. Findings are: During the environmental tour conducted on 1/9/13 at 1:25 PM, the phone numbers and addresses of the State Survey and Certification Agency and information needed to submit a complaint to the State Agency and the information on how to apply for and use Medicare and Medicaid were not posted. Interview with the Administrator on 1/9/13 at 1:25 PM verified the information was not posted and readily accessible to residents, employees and the public. Interview with the Social Services Director on 1/9/13 at 1:37 PM confirmed the information was not posted in the facility.	2015-06-01
