In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based on observation and interview; the facility staff failed to post daily nurse staffing information in the facility. This had the potential to effect 98 residents that resided in the facility. The facility census was 98. Findings are: Observation on 7/2/12 during the facility entrance initial tour revealed no daily nurse staffing information posted in any area of the facility. Interview on 7/2/12 at 10:18 AM with the Director of Nursing confirmed that the daily nurse staffing information had not been posted in the facility since January, 2012. |
