In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The facility census was 19 at the time of the survey and the survey sample size was 23. Findings are: A. Observation on 10/17/2012 at 9:15 AM revealed that posting of Nurse Staffing was documented on a dry erase board behind the Nurse's Station; however, the Nurse Staffing did not contain the facility name with the total number and actual number of nursing hours worked on that day. B. Observation on 10/18/2012 at 1:30 PM revealed that posting of Nurse Staffing was again documented on a dry erase board behind the Nurse's Station; however, there was no documentation of the facility name or the total number and actual number of nursing hours worked on that day. C. Observation on 10/23/2012 at 10:00 AM revealed that posting of Nurse Staffing was documented on a dry erase board behind the Nurse's Station; however, again the name of the facility and the total number and actual number of nursing hours were not posted. D. Interview on 10/23/2012 at 2:00 PM with the DON (Director of Nursing) revealed that a permanent record of Nurse Staffing was documented in a notebook; however, the DON confirmed that the total number and actual number of hours worked by nursing staff was not documented. E. Interview on 10/23/2012 at 2:35 PM with the DON revealed that the notebook with Nurse Staffing hours was kept in the filing cabinet under the Nurse's Station table and not accessible to residents or visitors. F. Review on 10/23/2012 of the Nurse Staffing notebook revealed that the name of the facility and the total number and actual number of hours worked by nursing staff was not documented in that record either. 2016-04-01
