In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Failure to honor resident choices related to sleeping and waking times does not allow the residents the right to choose their own schedule. Findings include: Review of the facility policy titled Resident Rights occurred on 10/17/19. This undated policy stated, . You have the right : To be offered choices and allowed to make decisions important to you, To expect the facility to accommodate individual needs and preferences . During an interview on 10/15/19 at 10:06 a.m., a family member (AA) stated, They (facility staff) get her (the resident) up way too early. There should be no reason she has to get up that early. Then I find her sleeping all the time with her head hanging down or leaned back in her wheelchair. She's up in her wheelchair from 5:30 (a.m.) until 8:30 (p.m.) when they put her to bed. During the group interview on 10/16/19 at 9:00 a.m. with residents identified by the facility as interviewable, Resident C stated staff get her up earlier in the morning than she chooses. If I don't get up they (staff) take my blanket and I get cold so I get up. Resident D stated staff make her get up earlier in the morning than she chooses. During a dressing change on 10/16/19 at 11:32 a.m. by an administrative nurse (#3), Resident #36 stated, Last night they (staff) wouldn't put me to bed, they made me sit here and sit here way over an hour and my back hurt. During an interview on 10/17/19 at 3:59 p.m., an administrative nurse (#1) stated she expected staff to allow residents to sleep in if they wanted. 2020-09-01
