Observation on 06/26/18 at 9:45 a.m. showed two certified nursing assistants (CNAs) (#8 and #9) assisted Resident #22 with perineal cares, however, failed to offer fluids upon completion of cares. Observation on 06/27/18 at 11:39 a.m. showed two CNAs (8# and #7) assisted Resident #22 with perineal cares, however, failed to offer fluids upon completion of cares.
