In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Based on observation, interview, and record review, the facility failed to ensure the care plan was updated to reflect the resident's current care needs and address fractured fingers on the resident's right hand, for 1 (#65) 19 sampled residents. Findings include: During an interview on 6/11/18 at 3:30 p.m. resident #65 stated she had fallen about a week or so ago while in the facility. She stated she broke two fingers on her right hand. During an observation on 6/11/18 at 3:30 p.m., resident #65 had an ace wrap around her right forearm, wrist, and fingers. Review of resident #65's care plan showed no information regarding the fractures fingers on the care plan. The care plan was dated 6/2/18. |
