Findings included: A facility policy titled, Perineal Care, dated (MONTH) 2014, revealed the perineal care steps for a female resident would include to wipe the rectal area thoroughly, including the labia and the buttocks using the same technique as the perineal area. During an observation, on 11/15/17 at 9:45 AM, Certified Nursing Assistant (CNA) #1 and CNA #2 performed peri-care on Resident #9. CNA #1 completed care over the front of the resident's perineum. CNA #1 and CNA #2 did not clean Resident #9's buttocks or the sacrum. When interviewed, on 11/15/17 at 9:45 AM, CNA #1 said she had completed care on Resident #9 and said she thought the policy was to clean the buttocks or the sacrum if the resident had an incontinent bowel movement. CNA #1 said she had completed training that included a check off for perineum care. CNA #1 confirmed she only washed the front perineal area for Resident #9. During an interview, on 11/16/17 at 10:15 AM, Registered Nurse (RN) #2, Staff Development Nurse, said the training for incontinent care included complete care which included washing the front of the perineum and the buttocks and sacrum after each incontinence episode regardless of a bowel movement. During an interview, on 11/16/17 at 11:00 AM, the Director of Nursing (DON) confirmed the policy was to complete the peri-care, the front and the back because of the risk for skin breakdown, infection, and the resident may be still wet. A review of the facility's face sheet revealed the facility admitted Resident #9 on 04/21/13. Resident 9's [DIAGNOSES REDACTED]. The Minimum Data Set (MDS), with an Assessment Reference Date (ARD) of 08/12/17, revealed staff assessed Resident #9 with severely impaired cognition. | 2020-09-01 |
