In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings: 1. Documentation in Resident #75's clinical record indicated that he/she was discharged /transferred to an acute care facility on 4/26/19 for treatment of [REDACTED]. The clinical record lacked evidence that the facility issued a written bed hold notice to the resident and/or legal representative. In an interview with the Licensed Social Worker (LSW) on 6/11/19 at 12:14 p.m. the surveyor confirmed that the clinical record does not support evidence that the bed hold notice was issued. 2. Documentation in Resident #13's clinical record indicated that he/she was discharged /transferred to an acute care facility on 5/16/19 for treatment of [REDACTED]. The clinical record lacked evidence that the facility issued a bed hold notice to the resident and/or legal representative. In an interview with the Licensed Social Worker (LSW) on 6/11/19 at 1:07 p.m., the surveyor confirmed that the clincal record does not support evidence that the bed hold notice was sent. 2020-09-01
