In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Finding: Documentation in Resident #39's clinical record indicated that the resident was transferred to the hospital on [DATE]. There was no evidence in the clinical record indicating that Resident #39 or the Resident's Representative was provided a written bed-hold notice that specified the duration of the bed-hold. On 6/12/18 at 1:53 p.m., during an interview with the Nurse Manager, a surveyor confirmed that there was no evidence that the bed hold notice was completed and provided to the Resident or Resident's Representative at the time of transfer to the hospital. 2020-09-01
