In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on record review and interviews, the facility failed to follow a physician order [REDACTED].#14). Finding: Documentation in Resident #14's clinical record, under the Therapy section, indicated the resident received physical therapy between 4/25/19 and 6/5/19. On 6/5/19, the Physical Therapist wrote that Resident #14's progress had plateaued and that nursing staff may resume the FMP for ambulation that was in place. Resident #14's primary physician signed this as an order to continue the FMP for ambulation. A review of the Resident #14's clinical record indicated there is no documented evidence that his/her FMP for daily ambulation is being followed. On 7/17/19 at 10:47 a.m., in an interview with the surveyor, Resident #14 stated he/she would like to ambulate everyday but staff are not doing it. On 7/17/19 at 2:40 p.m., in an interview with the surveyor, the Assistant Director of Nursing (ADON) stated Resident #14 does have a FMP for ambulation dated 5/29/19. The ADON was unable to find documentation in Resident #14's clinical record that the FMP was being followed. The ADON confirmed with the surveyor that the FMP for ambulation was not put in place on 6/5/19 and is not being followed. |
