In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings: 1. On review of Resident #46's clinical record, a surveyor noted the resident received Medicare Part A services that ended on 1/17/19 but the surveyor could not locate evidence that the required Skilled Nursing Facility Advance Beneficiary Notice (SNFABN) was provided to the resident so that he/she could make an informed decision to continue receiving the skilled services that may not be paid for by Medicare and assume financial responsibility. 2. On review of Resident #74's clinical record, a surveyor noted the Resident, received Medicare Part A services that ended on 1/14/19 but the surveyor could not locate evidence that the required Skilled Nursing Facility Advance Beneficiary Notice (SNFABN) was provided to the resident so that he/she could make an informed decision to continue receiving the skilled services that may not be paid for by Medicare and assume financial responsibility. On 2/26/19 at 11:15 a.m., in an interview with the Licensed Social Worker, the surveyor confirmed that SNFABNs were not issued prior to the end of Medicare Part A services. 2020-09-01
