In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings: 1. Documentation in Resident #21's clinical record indicated that he/she was discharged /transferred to an acute hospital on [DATE]. The clinical record lacked evidence that the facility issued a written discharge/transfer notice to the resident and/or resident representative. On 10/8/19 at 2:37 p.m., during an interview with the Director of Nursing, the surveyor confirmed that the clinical record did not contain evidence that the written transfer/discharge notice was given to the resident and/or resident representative. 2. Documentation in Resident #67's clinical record indicated that he/she was discharged /transferred to an acute hospital on [DATE]. The clinical record lacked evidence that the facility issued a written discharge/transfer notice to the resident and/or resident representative. On 10/10/19 at 10:09 a.m., during an interview with the Social Worker, the surveyor confirmed that the clinical record did not contain evidence that the written transfer/discharge notice was given. 2020-09-01
