In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Finding: On review of Resident #55's clinical record, the surveyor noted an admission date of [DATE]. When reviewing Resident #55's monthly Drug Regimen Review, the surveyor could not locate any monthly reviews completed prior to 5/2017. In an interview with the surveyor on 10/26/17 at 11:39 a.m., the 2 North Unit Clerk stated that the facility switched to a different pharmacy and when the new Pharmacist completed the initial Drug Regimen Review in May, he removed the reviews done by (the previous pharmacy) and the Unit Clerk, didn't know what he did with them. The Unit Clerk then searched the entire clinical record and could not find any Drug Regimen Reviews completed prior to (MONTH) (YEAR).
