In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

> Based on interviews and facility policy review, the facility failed to ensure all alleged violations involving misappropriation of resident property (funds) were reported timely (no later than 24 hours) to the State Survey Agency and evidence that alleged violations were thoroughly investigated for 1 of 3 residents (Resident #1). Finding: On 8/30/17 at 1:27 p.m., in an interview with Resident #1, he/she confirmed facility staff were made aware in (MONTH) of alleged missing funds in the amount of $240.00. On 8/30/17 at 11:35 a.m., in an interview with the Unit Nurse Manager, he/she acknowledged the alleged incident of Resident #1's missing funds; however, the Unit Nurse Manager could not provide evidence of a facility investigation into the missing funds, and further confirmed the allegation was not reported to the State Survey Agency. At the time of this interview, the Director of Nursing was present and he/she confirmed per regulation and their policy titled Abuse Prevention and Reporting, the facility failed to document an investigation and report the allegation to the State Survey Agency. |
